The Central New Mexico Community Access
Program (CAP-NM) aims to achieve 100% access
for all uninsured in Central New Mexico to a
primary care home. Health status is
improved through a collaboration and integration
between the six safety net providers in the
four-county target area. Particular attention
is focused on reducing health disparities
among vulnerable populations in these counties.
